Anterior cruciate ligament injury is the most common ligament injury in the knee. In a large number of patients, especially in partial ACL injuries or in acute cases, there is no clear correlation between the usual clinical tests and magnetic resonance imaging. The lever test was found to be effective in the diagnosis of ACL ruptures and is therefore considered a sensitive and simple test to perform in the diagnosis of ACL ruptures. Worldwide, the use of the free quadriceps tendon graft without a bone plug is not common. In the 1990s work by Fulkerson, Jokl and Shelton was published. These authors were the first to disseminate the technique of quadricipital grafting without a bone plug, with good results. This work corroborates the benefit of using this autograft. In recent years, the reconstruction of the LPFM has gained importance. The most commonly used technique is hamstring autograft. In some series, reports of complications are higher than 26%. This work aims to demonstrate the usefulness of the quadriceps graft, with a lower percentage of complications.
