Request a Blackwell Rewards Card

Please complete the form below to request a new Blackwell Rewards Card. The card will be posted to the address you provide (in step 2). Once your request has been processed we will also email your card number, allowing you to start collecting points online before your card arrives. If you are an existing Blackwell Online customer simply login with your email address and password below. If you are new to Blackwell Online please register below using your email address, and enter a password. Then click the 'Next step' button to enter your delivery address details for the card.
